MEMBERSHIPS: All memberships except standalone single month PIF (paid in full) start with a 3-month term commitment with payments to be debited from the payment method listed on then member's account once every 30 days.  At the end of the 3-month committed term, the memberships will become open ended with the same automatic debit payment cycle until cancelled. 

MEMBERSHIP UPGRADE/DOWNGRADE: Members can upgrade or downgrade a membership type anytime after the initial 3-month commitment once they enter the open end membership status. Use this form to send in your upgrade or downgrade request. these changes will take affect within 7 days from receipt of the request. 

CANCELLATIONS: SuperPHIT can cancel a membership for any reason without notice. The member can request a cancellation using this form. This will be the only cancellation method accepted. Cancellations will take affect on the 15th day following receipt of the cancellation request form.
